The King of Prussia Pharmacy Services will provide professionally competent pharmacy consultant services to our facilities that shall include:
• The provision of information, guided by Federal, State, ICF/MR, CHAP, and local codes, laws, rules, and regulations pertaining to medications, treatments, supplies, equipment, and services provided to the Facility.
• Monthly inspections of all areas in the Facility where medications are stored, dispensed and administered including documentation of problems and recommendations for correcting the identified problem(s) and improving medication administration practice standards.
• Conducting reviews of each resident’s medication regimen including the monitoring of pertinent laboratory data, checking the physicians’ orders against the MARs and TARs for the purpose of identifying any irregularities—a full written report of all pertinent findings, problems, irregularities and/or concerns with regard to pharmacotherapeutic regimens reviewed shall be submitted to the Medical Director and Director of Nursing.
• Availability to participate in on-going in-service or educational activities for the Facility staff including the maintenance of records showing the subject matter presented and a listing of the names of participants.
•Making himself/herself available for meetings as needs in the Facility may dictate.
